We are seeking an Aged Care Services Program Officer to facilitate the well-being and independence of older clients through comprehensive intake and assessment procedures. Your role is crucial in ensuring that older individuals receive tailored support to enable prolonged home living. By using your expertise in My Aged Care and employing a holistic approach, you connect clients with suitable services while addressing their diverse needs, including those of culturally and linguistically diverse backgrounds. Additionally, your contribution to the Support for Carers Program underscores your commitment to supporting caregivers, recognizing their invaluable role, and assisting them in maintaining balance and well-being while caring for loved ones.
This position is a 4.1 under the Social, Community, Home Care and Disability Services Award. It is a fixed term of 12 month role
