We are a leading provider of settlement services for refugees and migrants and provide a range of coordination and wrap around services for CALD communities across Melbourne.https://spectrumvic.org.au/service/aged-care-services/ABOUT THE POSITIONThe Program Officer supports and maintains intake & assessment, aged care services funded programs including monitoring and managing the My Aged Care (MAC) Portal for referrals.The Program Officer will be working across aged care programs including Support for Carers Program (HACC funded), Care Finder, CHSP Individual services (Personal Care and Flexible respite services) and will work collaboratively within the Aged Care team.  The position of the Program Officer is to check eligibility and support clients to navigate and access entry level service provision.  The role involves developing goal directed care plans and identifying and administering appropriate service options.  Funded programs are provided by Spectrum in a variety of ways that meet client needs including short term services, in-home day and centre based, access to housing assistance and community access activities.  ABOUT YOUThis role will bring together your experience and understanding of people leadership, quality, and service improvement to ensure the delivery of valued services and outcomes aligned with Spectrum’s vision and strategy and driven by our community needs.To be considered for this position, you will haveDiploma in Community ServicesExperience in the aged and / or disability sectorsKnowledge of Aged Care Programs and Support at Home ModelPossess previous experience in CHSP, the Home Care Package program, and/or relevant aged care administrative roles.Experience or knowledge of CHSP or HACC programs preferably in a case worker or service coordination roleUnderstanding of the Service Coordination Framework including initial contact, needs identification, assessment, care planning, review and linking to aged care quality standards.WHY SPECTRUM?At Spectrum, each member of our team is valued, we strive to support people to grow both personally and professionally.
