This role is designed to support the seamless growth and swift onboarding of HCP clients.The Aged Care Intake Liaison Officer will be at the forefront of establishing and maintaining a robust database of potential HCP clients. The core duty involves expediting the onboarding process for new HCP client referrals, both from existing and new sources. This entails thorough engagement with clients to gather essential details required for their assessment and the coordination of tailored services.ABOUT YOUThis role will bring together your experience and understanding of people leadership, quality, and service improvement to ensure the delivery of valued services and outcomes aligned with Spectrum’s vision and strategy and driven by our community needs.To be considered for this position, you will haveHold a relevant tertiary qualification or a minimum Diploma level in Social Work, welfare, or a related field pertaining to community and aged care servicesExperience in the aged and/ or disability sectorsKnowledge of Home Care Package Servicing ModelPossess previous experience in CHSP, the Home Care Package program, and/or relevant aged care administrative roles.WHY SPECTRUM?At Spectrum, each member of our team is valued, we strive to support people to grow both personally and professionally.
