Funded programs are provided by Spectrum in a variety of ways that meet client needs including short term services, in-home day and centre based, access to housing assistance and community access activities. Spectrum has a key focus on clients who are from a culturally and linguistically diverse (CALD) background, and the Program Officer will need to carefully consider the diverse needs of our clients in planning and service delivery.Position PurposeThe Program Officer supports and maintains MAC Intake & Assessment, Aged care services funded programs including monitoring and managing the My Aged Care (MAC) Portal for referrals. The Program Officer will be working across aged care services program including Support for Carers Program (HACC funded), Care Navigation, Assistance with Homelessness/ Hoarding (Care finder), CHSP Access and Support for above 65 & CHSP Individual services (Personal Care and Flexible respite services) and collaboratively working within the Aged Care team. Key DeliverablesComplete the MAC Intake & Assessment, eligibility with Commonwealth Home Support Programs (CHSP) & Home, and Community Care Program (HACC), Care Finder program requirements and guidelines (Primary health network)Provide face-to-face and phone interviews for clients with enquiries for aged care and disability services.Determine eligibility and conduct assessment for eligibility into the program.Triage and assign referrals and support plan reviews for assessment.Meet key performance indicators including delivery of a specified number of clients and carers supported, as per funding agreements and program’s KPIGenerate and provide accurate client’s information via gold care and support the reporting requirements for Data Exchange (DEX), and internal purposes as required.Maintain accurate, professional client records and updates documentation in the CMS (Gold Care)Promote the service and provide information about Spectrum’s Aged Care Services programs and across Spectrum departments and to targeted communities.About youCertificate III or IV in Individual Support or similarExperience in the aged and / oHave a knowledge of home care compliance - specifically CHSP and Community carer disability sectorsHave a knowledge of home care compliance - specifically CHSP and Community careExperience or knowledge of CHSP or HACC programs preferably in a case worker or service coordination roleUnderstanding of the Service Coordination Framework including initial contact, needs identification, assessment, care planning, review and linking to aged care quality standards.If you believe that this opportunity is a match for you, please click on the apply button and provide a cover letter addressing the key selection criteria, your CV, and a copy of your qualifications.Spectrum actively encourages and welcomes applications from all candidates.
